A diagnosis of Parkinson\u2019s disease is heavy for both the individual and their family. There is a lot to learn, and suddenly the future you imagined looks very different. Most conversations with physicians focus on tremor, stiffness, medication schedules, and mobility challenges. What many families do not hear about right away are the speech changes in Parkinson\u2019s disease<\/strong> and how they affect everyday communication over time.<\/p>\n\n\n\n

These communication changes can be confusing and frustrating for spouses, children, and caregivers. Families often ask questions like: Why do people with Parkinson\u2019s speak quietly? Why does it seem like my spouse with Parkinson\u2019s mumbles?<\/em> The answer lies in the neurological changes occurring in the brain.<\/p>\n\n\n\n

Parkinson\u2019s disease gradually affects the basal ganglia and the neural pathways responsible for motor control <\/strong>because of the progressive loss of dopamine-producing neurons in the brain, which disrupts the signals that coordinate movement and speech<\/a>. While we often think of this as affecting walking or tremor, the same motor systems control speech, breathing, and facial movement. Over time, these neurological changes lead to Parkinson\u2019s communication changes, including soft speech, reduced clarity, and slower articulation.<\/strong><\/p>\n\n\n\n

As an audiologist, I often see these effects firsthand when couples come into the office struggling to understand each other. One spouse may assume hearing loss is the main issue, while the other believes their partner simply is not speaking clearly. In reality, the problem is usually far more complex, and understanding it can help families navigate communication challenges with more patience and compassion.<\/p>\n\n\n\n

One afternoon, I was sitting in my office speaking with a patient who wore hearing aids but struggled to hear her husband clearly. Her husband also had hearing loss and Parkinson\u2019s disease, but he did not consistently wear hearing aids.<\/p>\n\n\n\n

She explained, \u201cHe mumbles, and unless I\u2019m standing right in front of him, I can\u2019t understand him \u2014 even with my hearing aids. I think it\u2019s because he isn\u2019t wearing his aids.\u201d<\/p>\n\n\n\n

If only it were that simple.<\/p>\n\n\n\n

His Parkinson\u2019s mumbling speech<\/strong> was likely not caused by hearing loss alone. Instead, it was most likely due to speech changes in Parkinson\u2019s disease<\/strong>, a common but often overlooked symptom.<\/p>\n\n\n\n

Parkinson\u2019s disease causes a motor speech disorder known as hypokinetic dysarthria<\/strong>, affecting nearly 70\u201390% of individuals with the condition (Duffy, 2019)<\/em>. This disorder results from changes in motor control caused by dopamine loss in the brain.<\/p>\n\n\n\n

Several factors contribute to communication problems in Parkinson\u2019s disease<\/strong>:<\/p>\n\n\n\n

Reduced breath support<\/h3>\n\n\n\n

Speech requires airflow. As Parkinson\u2019s progresses, posture often changes and people begin to lean forward or slouch. This posture reduces lung expansion and limits breath support.<\/p>\n\n\n\n

When there is less air available to power the voice, speech becomes quieter. This is why Parkinson\u2019s soft voice<\/strong> is one of the earliest communication symptoms.<\/p>\n\n\n\n

Additionally, the disease can affect diaphragm coordination<\/strong>, further limiting the force needed to project speech.<\/p>\n\n\n\n

Changes in posture and vocal direction<\/h3>\n\n\n\n

Many individuals with Parkinson\u2019s gradually begin speaking downward toward their lap rather than projecting their voice outward toward the listener.<\/p>\n\n\n\n

This subtle shift means their speech energy is directed away from the person they are trying to communicate with. Even when the voice is audible, it may not be clear enough for the listener to understand.<\/p>\n\n\n\n

Tongue and articulation difficulties<\/h3>\n\n\n\n

Another important factor involves the tongue and oral muscles.<\/p>\n\n\n\n

These muscles play a major role in both swallowing and speech. As Parkinson\u2019s affects muscle control, articulation difficulty<\/strong> becomes more noticeable.<\/p>\n\n\n\n

For example, the \u201ct\u201d sound requires the tongue to briefly touch the roof of the mouth behind the front teeth. If the tongue cannot move precisely or quickly enough, the sound becomes unclear.<\/p>\n\n\n\n

Over time, these small articulation problems accumulate, leading to Parkinson\u2019s voice changes over time<\/strong> and reduced speech clarity.<\/p>\n\n\n\n

Facial masking and reduced expression<\/h3>\n\n\n\n

Parkinson\u2019s can also reduce facial movement, a symptom called facial masking<\/strong>. When facial expressions decrease, communication becomes more difficult because listeners rely heavily on visual cues.<\/p>\n\n\n\n

Without those cues, conversations require more concentration from both people.<\/p>\n\n\n\n

Cognitive load and processing changes<\/h3>\n\n\n\n

As Parkinson\u2019s progresses, communication can also become harder due to cognitive load<\/strong>.<\/p>\n\n\n\n

The brain must coordinate breathing, vocalization, articulation, and language all at once. When neural pathways are disrupted, these processes require more effort.<\/p>\n\n\n\n

This increased cognitive demand can lead to slower speech, pauses, or difficulty organizing thoughts.<\/p>\n\n\n\n
